Step 1: Know the medication concentration

Each medication has a different concentration, which can be found on its medical leaflet. For example, paracetamol has a concentration of 200 mg/mL, which means there are 200 mg in every 1 mL of the medication. Some medications are diluted before use, and in such cases, the concentration of the solution after dilution should be considered. We won't delve deeper into the topic of dilution here, as we will have a dedicated post on how to dilute medications and calculate the concentration after dilution. For the purpose of this step-by-step guide, let's assume a hypothetical solution with a concentration of 1 mg/mL and a volume of 200 mL.

Step 2: Know the medication dose

In the case of the hypothetical medication, the sedation dose ranges from 30 to 200 mcg/kg/h. These values should be consulted in the package insert, books, or other reliable sources. In this case, multiply this dose by the patient's weight in kilograms. Assuming a patient weight of 70 kg and a dose of 30 mcg/kg/h, we will need 30 mcg x 70 kg = 2100 mcg/hour to initiate the sedation for this patient.

Step 3: Calculation of dose in mL/hour

To calculate the dose in mL/hour (as used in the infusion pump), it is necessary to first convert micrograms (mcg) to milligrams (mg). Therefore, 2100 mcg is equivalent to 2.1 mg (divide by 1000). To provide 2.1 mg/hour to the patient, using the hypothetical solution of 1 mg/mL, we apply the following rule of three:

2.1 mg ----- X (mL)

1.0 mg ----- 1 mL

X = (2.1 mg X 1 mL)/ 1mg = 2.1 mL

Therefore, we need 2.1 mL/hour of the hypothetical solution with a concentration of 1 mg/mL to provide 2100 mcg/hour, which is equivalent to 30 mcg/kg/hour for a 70 kg patient.

The calculation of time can be done using the following rule of three:

200 mL ----- X (hours)

2.1 mL ----- 1 hour

X = (200 mL X 1 hour)/ 2.1 mL = 95.2 hours

The hypothetical solution with a volume of 200 mL and an infusion rate of 2.1 mL/hour would last for 95 hours.
